ATTENTION!!!! All students interested in playing sports this season must have a PHYSICAL EVALUATION FORM on file with our Athletic Department. You can come by the Field House Trainers Office to pick up all eligible forms. Physical Evaluation must be completed yearly and must be on file before participation in any sport.
